MTV invoked a lifetime ban on Andrew Dice Clay and 22 years later in walks foul-mouthed, chain-smoking Jo Calderone. Lady Gaga again pulled off the unexpected as Jo, in what seemed like a musical ode to "A Streetcar Named Desire's" Stanley Kowalski.
